The maturity model

Five levels. Scored against your own operation.

Every safety programme is somewhere on this staircase. The model tells you which step you are on and what the next one looks like. It compares you with yourself, not with an industry average.

  1. Level 1FoundationThe basics exist on paper.
  2. Level 2ControlledThey are followed, and you can prove it.
  3. Level 3SystematicThe same way every time, across every site.
  4. Level 4ProactiveYou find problems before they find you.
  5. Level 5LeadingThe system predicts risk. People still review and decide.
Most businesses start at level 1 or 2. Level 3 is where customers stop asking questions.
Five areas, weighted

Leadership and hazard control count the most, because everything else depends on them.

Twenty-five questions across five areas. Each area gets its own level, so you can see exactly where you are strong and where you are not.

  1. 25%Organisation & Leadership
Who owns safety, and what leadership actually sees.
  2. 25%Hazards & Risk
Whether you know what can hurt someone, and what you did about it.
  3. 20%Incidents & Corrective Action
What happens after something goes wrong, or nearly does.
  4. 20%People, Training & Culture
Whether your people are competent, and whether they tell you things.
  5. 10%Compliance, Records & Reporting
Whether you can find the evidence when somebody asks for it.
What you get

A level, five area scores and your three biggest gaps.

Each question is answered no, partly or yes. Some questions describe habits you want to leave behind, so saying yes to them lowers the score. Honest answers are the only useful ones.

Three layers

The paperwork, the work itself, and the dial leadership reads.

Eighteen tools in three layers, lined up with how recognised safety management systems are built. You add them as your plan calls for them.

Plan

Foundation

Policies, roles, obligations and controlled documents. The things an inspector asks for first.

  • Legal register and compliance calendar
  • Governance and scope
  • Committee and worker participation
  • Policy and objectives
  • Roles and accountability
  • Documents and records
Do

Execution

Hazards, training, permits, contractors, emergencies and incidents. The work as it actually happens.

  • Hazards and risk
  • Competence and training
  • Communication
  • Permits and controls
  • Management of change
  • Contractors
  • Emergency preparedness
  • Incidents and near misses
Check and act

Intelligence

Monitoring, audits, reviews and the insights that tell you what is drifting.

  • Monitoring and compliance
  • Internal audit
  • Management review
  • Insights and improvement
Two numbers

The score you give yourself, and the score your records give you.

Your Culture Check sets the first number. Once your team works in the platform, the same questions are answered from your own hazards, incidents, actions, training, documents and reviews. That is the second number. They sit side by side and are never blended.

Where something cannot be measured yet, it says so. Where it needs a tool you have not added, it says that too.

What each line can say

  • Met Your records back it up.
  • Partly met Some of what is expected is there.
  • Not met Records exist but do not back it up.
  • No records Nothing to read yet.
  • Not measured yet A later level, not scored.

Find your level.

Fifteen minutes, twenty-five questions. You get a level, a score for each area and your three biggest gaps.
